Passpoart and or passpart card.
The passport book and the passport card are $120(application fee:$95, execution fee $25)
Shot record: if you are taking any medication make sure you speak to your doctor
and talk about any refills needed.
Birth certificate
Include other documents that may be needed in case of emergency; Ex.
loss of a document or stolen.
